Equipment Loan Borrower Agreement

By borrowing any equipment from Old Dominion University Libraries, you agree to be bound by the terms in our Equipment Loan Borrower Agreement and Equipment Loan Policy.

Need Equipment for a Class Assignment?

Before submitting an Equipment Loan Exception Request Form, please read the following information:

  • Equipment exception requests are answered Monday-Friday (excluding holidays and university closings).
  • Equipment requests should be submitted at least a week prior to when the equipment is needed.
  • Please understand that this is a request and does not guarantee equipment will be set aside for you.
  • This form is designed for patrons to request equipment needed to complete a specific class assignment.  This form is not designed to request equipment for general coursework.
  • While the Learning Commons considers requests from student organizations registered with the Office of Student Engagement and Traditions and campus offices, students in need of equipment to complete a class assignment will always have priority.  It is recommended that faculty and staff contact their departments for their equipment needs.
  • This form is designed to provide equipment needed for academic reasons.  Requests will not be approved if certain equipment is required as a condition of a class (i.e. a class requires students to have a camera).
  • Equipment exception requests will only be approved if the form is fully filled out and supporting documentation is attached.
