Electronic Forms (Efffective Tuesday, May 14 2019)
Starting on 5/14/19, the following forms will only be available electronically. If the ODU web page ever shuts down, downtime forms will be kept in the white file cabinet in room 1307.
Directions for patrons: University Libraries' homepage > click More..., under Services > click non-electronic materials on reserve, under Course Reserves > click Submitting a Course Reserve tab
This form should only be used if Alma is down but ODU’s web pages are still up. If Alma and ODU’s pages are down, you should use the paper equipment loan downtime form
