Need Equipment for a Class Assignment?
All of the equipment featured in this guide is intended for educational purposes and is typically loaned on a first come, first serve basis. If you are in need of equipment to complete a class project or class assignment, please submit an Equipment Loan Exception Request Form. It is highly recommended that the form is submitted at least a week before equipment is needed. Please understand that this is a request and does not guarantee equipment will be set aside for you. All equipment loan exception requests will be answered by the next business day at 5:00pm.
In order to have your request considered, you must email a copy of your syllabus or a class assignment description from your professor to email@example.com after submitting this form. Please put the course number of the class in the subject of the email. The information on the syllabus or class assignment description must match the information provided in the Equipment Loan Exception Request Form. Due to limited equipment, the request will not be considered if the syllabus or class assignment description from your professor is not emailed.
Equipment Loan Agreement Form
All ODU students, faculty, and staff MUST agree to the terms and conditions in the Equipment Loan Agreement Form. This form must be completed once per academic year before equipment is checked out.
